Facility Use Request

REQUESTS FOR JANUARY-DECEMBER 2022
All clubs and committees must do their own set-up and clean-up for their events and meetings. The facility/furniture must be returned in the condition and placement in which it was originally found. Please be sure to empty your trash; only bagged trash goes in the blue trash bins at the Club House, no loose trash, please.

EXISTING SANCTIONED AVPOA CLUBS
This form will act as a renewal of the club's desire to remain an active sanctioned club for the new year.
We ask that you provide a roster of members* (acceptable files: word document, spreadsheet, or pdf) to include their name, phone, email address, and an emergency contact that can be attached to this submission form. You can also email me your list directly if you so choose to [email protected]
